01. Privacy Policy
Information We Collect
When you visit our website, contact our practice, or inquire about services, we may collect information you voluntarily provide, including your name, email address, telephone number, and the information you choose to include in a message or inquiry.
Our website and its service providers may also collect certain technical information, such as your browser type, device information, pages visited, and general website usage data.
We use this information to respond to inquiries, facilitate website functionality, understand how visitors use our website, and improve our services.
How We Use Your Information
Information submitted through our website may be used to respond to requests for information, communicate about scheduling or services, direct you to appropriate resources, maintain website functionality, and comply with applicable legal obligations.
We do not use information submitted through our website to establish a therapeutic relationship automatically.
We do not sell personal health information. Any additional website analytics, advertising, or tracking practices must be disclosed in accordance with the technologies actually used by the practice.
Website Forms and Email Communication
Please avoid including highly sensitive medical information, detailed mental health histories, or confidential clinical records in general website contact forms.
Although we take privacy seriously, ordinary email and general website communications may not provide the same protections as a designated secure patient portal.
Submitting a contact form or sending an email does not establish a therapist-client relationship. A therapeutic relationship begins only after appropriate intake procedures, informed consent, and acceptance into care.
Cookies and Website Analytics
Our website may use cookies or similar technologies necessary for website functionality, security, and performance.
Additional analytics or tracking tools, if enabled, may collect information about how visitors interact with the website.
You may be able to manage cookies through your browser settings. Disabling certain cookies may affect website functionality.
Third-Party Services
Our website is hosted through Squarespace and may contain links to third-party websites, scheduling systems, or other external services.
These providers may maintain their own privacy policies and security practices. Mun & Mind Therapy is not responsible for the content or privacy practices of independently operated websites.
Where third-party services process protected health information on our behalf, appropriate privacy and security arrangements must be maintained as required by applicable law.
Information Security and Retention
We take reasonable administrative, technical, and organizational measures to protect information under our control.
However, no internet-based communication or electronic storage system can be guaranteed completely secure.
Information is retained and disposed of according to applicable legal requirements, professional obligations, and the practice's established record-retention policies.
Your Privacy Rights
Depending on the type of information involved and applicable law, you may have rights to request access to information, request corrections, ask questions about how information is used, or raise privacy concerns.
Rights relating to protected health information are described more fully in our HIPAA Notice of Privacy Practices.
Children's Privacy
This website is intended primarily for adults seeking information about mental health services for themselves or their families.
Parents and legal guardians should contact the practice directly regarding services for minors. Confidentiality, consent, and access to records involving minors are governed by applicable law and clinical policies.
02. HIPAA & Clinical Confidentiality
Protection of Health Information
Mun & Mind Therapy recognizes the importance of maintaining the confidentiality of information shared in a therapeutic relationship.
When applicable, protected health information is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA), relevant state laws, and professional ethical requirements.
Clinical records, treatment information, and communications related to care may be subject to additional protections beyond those applicable to general website inquiries.
Limits of Confidentiality
Therapy is generally confidential, but confidentiality is not absolute.
Depending on applicable law and the circumstances, disclosure may be permitted or required in situations involving suspected abuse or neglect, certain serious threats to safety, court orders or other lawful processes, and other legally authorized circumstances.
Your therapist will review confidentiality, its limitations, and your rights during the informed-consent process.
Notice of Privacy Practices
Clients have the right to receive the practice's official Notice of Privacy Practices, which explains how protected health information may be used and disclosed and describes applicable patient rights.

03. Website Terms of Use

The information provided on munmindtherapy.com is intended for general educational and informational purposes.
Website content may describe mental health concerns, therapeutic approaches, clinician specialties, and available services. It is not intended to provide individualized diagnosis, treatment, or professional clinical advice.
No Therapist-Client Relationship
Browsing this website, completing a contact form, or communicating with the practice does not automatically establish a therapist-client relationship.
Services are provided only after the practice determines that appropriate care can be offered and the necessary intake and consent requirements have been completed.
No Guarantee of Outcomes
Therapy is an individualized and collaborative process. Experiences, progress, and outcomes vary.
Information on this website should not be interpreted as a promise or guarantee of any particular therapeutic result.

We make reasonable efforts to keep website information current and accurate.
However, clinician availability, licensing, accepted insurance plans, service offerings, and other practice details may change.

04. Clinical Services & Telehealth Disclosures
Availability of Services
Mun & Mind Therapy is a Chicago-based mental health practice offering services to eligible clients in Illinois.
Services may include individual therapy, couples counseling, family therapy, maternal mental health support, parent training, and professional consultation, depending on clinician qualifications and availability.
Not every service is offered by every clinician.
Telehealth Services
Telehealth involves providing clinical services through electronic communication technologies.
While telehealth can make therapy more accessible, it may involve limitations, including technical interruptions, privacy considerations, and situations in which remote care may not be clinically appropriate.
Clients receiving telehealth services must meet applicable location, consent, and clinical eligibility requirements.
Licensing and PSYPACT
Clinicians may provide services only where legally authorized to practice.
Certain licensed psychologists at Mun & Mind Therapy may be authorized to provide interstate telepsychology through PSYPACT, subject to their current authorization and the requirements of participating jurisdictions.
PSYPACT authorization does not automatically apply to every clinician, every service, or every state.
Clients must provide accurate information about their physical location during appointments. Services may be unavailable if the clinician is not authorized to provide care in that location.
Emergencies and Crisis Situations
Mun & Mind Therapy's website, contact forms, and general email addresses are not monitored as emergency response services.
If you are experiencing an immediate emergency or believe that you or another person is in immediate danger, call 911 or visit the nearest emergency department.
In the United States, you may also call or text 988 to reach the Suicide & Crisis Lifeline.
Please do not use website contact forms to communicate urgent safety concerns.
Fees, Insurance, and Appointments
Fees, insurance participation, payment responsibilities, cancellation policies, and other financial arrangements are addressed through the practice's current financial policies and intake documentation.
Insurance coverage is not guaranteed. Clients are responsible for understanding their plan benefits, including deductibles, copayments, coinsurance, and applicable coverage limitations.
Contact the practice for current fee and insurance information before beginning services.
05. Good Faith Estimate Notice
Your Right to Receive a Good Faith Estimate
Under the federal No Surprises Act, individuals who do not have health insurance or who choose not to use their insurance for healthcare services generally have the right to receive a Good Faith Estimate explaining the expected cost of care.
A Good Faith Estimate provides information about anticipated charges for scheduled or reasonably expected healthcare services.
You have the right to receive a Good Faith Estimate before receiving eligible services.
Who Is Eligible?
You may be eligible for a Good Faith Estimate if you do not have health insurance or if you have insurance but do not intend to use it to pay for the services you are requesting.
You may request an estimate when scheduling services or when asking about the expected cost of care.
When Will I Receive My Estimate?
For eligible uninsured or self-pay individuals:
If services are scheduled at least 3 business days in advance, an estimate generally must be provided within 1 business day after scheduling.
If services are scheduled at least 10 business days in advance, an estimate generally must be provided within 3 business days after scheduling.
If you request an estimate without scheduling services, an estimate generally must be provided within 3 business days.
What Does the Estimate Include?
Your personalized Good Faith Estimate will describe the services reasonably expected to be provided, applicable service codes, expected charges, and other required information.
The estimate may reflect the anticipated frequency and duration of treatment based on the information available at the time it is prepared.
Because mental health treatment is individualized, the actual number of sessions or services may change.
A Good Faith Estimate is not a bill, a guarantee of final charges, or a requirement that you receive treatment.
What If My Bill Is Higher Than the Estimate?
If you receive a bill from a provider or facility that is at least $400 more than the Good Faith Estimate provided by that provider or facility, you may be eligible to initiate the federal Patient-Provider Dispute Resolution process.
Generally, you must begin the dispute process within 120 calendar days of receiving the initial bill.
Starting an eligible dispute will not adversely affect the quality of healthcare services provided to you.
